NEW PATIENTS

Your path to
better mental health.

A clear, reassuring guide to starting your journey with Mountain States Psychiatry.

GETTING STARTED

One step
at a time.

For adults and adolescents ages 12 and older, initially in Colorado. These steps describe the planned intake experience; appointment requests and clinical intake are not enabled in this preview.

01

Intake Request

Once the approved intake pathway is available, begin by requesting an appointment through that confidential process or contacting the practice for instructions. Basic information will help clarify your needs and confirm Colorado availability.

Do not submit medical details or documents through this preview or ordinary email.

02

Clinical Screening

Your information will be reviewed to consider whether outpatient telehealth is appropriate for your circumstances. Screening helps identify a suitable care pathway and whether different or additional support may be needed.

03

Initial Consultation

If the practice is an appropriate fit, the next step is a 60-minute initial psychiatric evaluation. This first conversation focuses on connection, understanding your concerns, and developing an individualized, trauma-informed plan.

General initial evaluation: 60 minutes · $375 self-pay.

PREPARING FOR YOUR VISIT

Required documents
& information.

Have the following available for your intake visit. Final instructions will explain what to provide and how to share it through the approved clinical workflow—not this preview.

01

Photo Identification

Prepare appropriate photo identification, such as a driver's license, passport, or state-issued ID, for identity verification. For adolescents, the practice will explain the patient and parent or guardian verification and consent requirements before the visit.

02

Complete Medication List

Include names, doses, and prescribing clinicians for all current medications, along with supplements. This helps provide a clear picture of your existing treatment.

03

Medical & Psychiatric History

Prepare relevant records or summaries from prior medical and mental health care when available. Instructions for any authorized records transfer will be provided through the approved intake process.

04

Privacy & Consent Forms

Review the privacy information, applicable acknowledgments, and telehealth and treatment consent forms provided before the appointment. The final intake pathway and required forms are being configured.

WHAT TO EXPECT

Time to understand
your story.

Your general initial psychiatric evaluation is a 60-minute diagnostic session.

This time is dedicated to understanding your history, current challenges, and goals. We will discuss your questions and treatment preferences as we consider an individualized plan together.

For a virtual appointment, arrange a quiet, private space and a reliable internet connection. Have a camera- and microphone-enabled device ready; headphones can help with sound and privacy.

Specialized ADHD evaluations are a separate appointment type: 75 minutes at $475 self-pay. This does not change the 60-minute length of general initial psychiatric evaluations.
